چکیده

Sarcoidosis reflects a multisystem granulomatous disease of unknown etiology that is characterized by the formation of non-caseating granuloma in various organs. Cardiac involvement may occur in one out of four patients accounting for an increased mortality rate in these patients. F-fluorodeoxyglucose-positron emission tomography( F-FDG-PET)is increasingly applied for the detection and characterization of systemic and cardiac sarcoid disease. In particular, F-FDG-PET has the unique potential to identify early and inflammatory cardiac sarcoid disease before developing structural alterations in the myocardium such as fibrosis and/or scarring. F-FDG-PET is not only suitable for early disease identification but also for installing and guiding immune-suppressive therapy to improve cardiovascular outcome that, however, needs to be further tested in large-scale clinical trials. FFDG-PET, is likely to introduce an image-guided and personalized preventive medicine approach in sarcoid patients in the near future.

Pericardial sarcoidosis presenting as PUO diagnosed on FDG PET CT scan

Pyrexia of unknown origin (PUO) is a common problem in day-to-day practice. FDG PET CT is an established investigation that aids in identifying the cause of PUO. Due to its high sensitivity PET detects an occult hypermetabolic focus in the body where CT helps in anatomical localization, vascularity, enhancement characteristics of the lesion detected on PET. It helps to differentiate benign vers...

(18)F-FDG-PET CT findings in a patient with early-phase cardiac sarcoidosis.

A 54-year-old man with clinically and pathologically diagnosed cutaneous, ocular and pulmonary sarcoidosis was referred to the Cardiology Department for a prolonged PR interval and intermittent advanced atrioventricular block (Picture 1A and B). Echocardiography, gallium-67 scintigraphy and cardiac magnetic resonance imaging revealed no evidence of cardiac sarcoid involvement. F-fluoro-2deoxygl...

Cardiac MRI vs. myocardial 18F-FDG PET/CT in patients with clinical concern for cardiac sarcoid

Methods 168 patients had both CMR and myocardial PET/CT studies for suspected cardiac sarcoid at a single institution from 11/1/2009 to 9/5/2014. 106 had both studies performed within 3 months; 2 MRIs were nondiagnostic, leaving 104 patients for analysis, mean age 57 (18-80), 33 females and 71 males. The presence of DE on CMR and FDG uptake on PET/CT was compared and stratified by the time betw...

Imaging cardiac sarcoidosis by cardiac positron emission tomography (PET): a local experience using a high-fat, low-to-no carbohydrate protocol.

Dear Editor, A 40-year-old female presented acutely with syncope and recurrent ventricular tachycardia (VT) requiring repeated cardioversion. Transthoracic echocardiogram (TTE) revealed mildly depressed systolic function of 40% to 45% with normal right ventricular size and function. Coronary angiogram revealed insignifi cant coronary artery disease (CAD). Biopsy of enlarged mediastinal lymph no...

Cardiac MRI and FDG-PET in the diagnosis of cardiac sarcoidosis

Methods 30 patients meeting Japanese Ministry of Health & Welfare guidelines [2] for clinical cardiac sarcoidosis were investigated with FDG-PET CT (Gemini TF Philips) and CMRI (Aera 1.5T Siemens) on the same day. Patients undergoing FDG-PET followed a 24 hour low-carbohydrate diet and overnight fast [3]. CMRI examination included SSFP assessment of left ventricular (LV) function, short axis T2...
